Possibly due to your front-end templates: dcc01 (1.2.0) 1 | t4_bs5_blank (1.2.0)
Are you actively using either of these? The best advice is to set the default front-end template to Cassioepia. Once the migration is done successfully you can set the front-end template to your choice, and re-set the modules correctly for that template.

So saying the above, if you're using the T4 template, make sure you have the latest release for J4/J5 and the relevant T4 framework plugin. Your FPA doesn't list that plugin; I would expect it to have done so.
